Service Warranty Policy at Nhan Tam Dental Clinic

I. Warranty Conditions

1.1 General Warranty Conditions

• Warranty Forms: Warranty Card and Electronic Warranty
• The customer must be using products or services from Nhan Tam Dental Clinic that are still within the warranty period.
• The customer must present a valid warranty card that is intact, unaltered, and matches the data managed by Nhan Tam Dental Clinic’s system.
• The customer must attend follow-up appointments, undergo X-rays (if required), and maintain oral check-ups and cleanings every 6 months.
• The customer must follow proper at-home dental care as instructed by the dentist.

1.2 Cases Not Covered by the Warranty

• Failure to maintain proper oral hygiene (e.g., smoking, improper cleaning).
• Failure to attend follow-up appointments as instructed.
• Occurrence of new medical conditions after treatment at the clinic that may affect oral health (e.g., diabetes, osteoporosis, cancer radiation therapy, epilepsy, bruxism, etc.).
• Dental trauma caused by accidents, sports, or other external factors.
• Natural degeneration of teeth or gums.
• Previous dental treatments not performed at Nhan Tam Dental Clinic.
• Patients requesting to alter treatment outcomes different from the original plan agreed upon with the dentist.
• Treatments requested by the patient despite being advised by the dentist that the teeth may be unstable, fragile, or less durable.
• Any intervention performed by another dentist before returning to Nhan Tam Dental Clinic.

1.3 Warranty Card Information

After completing treatments such as Implants, Dental Crowns, Orthodontics, Fillings, or Dentures at Nhan Tam Dental Clinic, patients will be issued a warranty card corresponding to their chosen treatment. The warranty card includes the patient’s full name, warranty period, tooth number(s), and restored tooth position. In addition, the card contains a unique serial code that allows patients to easily check their full warranty details on Nhan Tam Dental Clinic’s official website.

II. Warranty Policy and Conditions for Implant Services

2.3 Warranty Period for Implant Placed at Another Clinic

• Titanium Implant Crown: Free repair or replacement under warranty conditions within 2 years
• Cercon Implant Crown: Free repair or replacement under warranty conditions within 5 years
• Cercon HT / E.max Implant Crown: Free repair or replacement under warranty conditions within 3 years. In the 4th year, if the crown has issues such as cracking, chipping, or loosening, a 20% discount will be applied. In the 5th year, a 10% discount will be applied
• Lava Esthetic (3M – USA) / Lisi (GC – Japan) Implant Crown: Free repair or replacement under warranty conditions within 3 years. From the 4th to the 5th year, if the crown has issues such as cracking, chipping, or loosening, a 20% discount will be applied. In the 6th year, a 10% discount will be applied

NOTE: After the warranty period: Treatment will be charged as a new regular case

2.4 Warranty Period for Full-Arch Removable Implant

2.5 General Terms

The success rate of dental implants is very high, ranging from 97% to 100%. However, there remains a small percentage (below 3%) of implants that fail to integrate with the bone due to various factors. In such cases, the clinic will reinsert a new implant (free of charge) at the same position. If the patient prefers a refund instead of continuing treatment, 50% of the implant fixture cost already paid will be refunded. This refund applies only to the implant fixture and excludes costs for bone grafting, PRF membrane, and temporary teeth (if any).

In the case of reimplantation, the clinic will replace the failed implant with a new implant of the same type free of charge. If the patient wishes to upgrade to a higher-quality implant, they will pay the price difference between the two implant types.

If the patient’s crown becomes cracked or broken within the warranty period, Nhan Tam Dental Clinic will replace it entirely free of charge.

If damage occurs beyond the warranty period, the crown will be charged according to the current clinic price list at that time. However, patients will receive a discount depending on the extent of damage to the crown or bridge.

2.6 Warranty Policy

Purchase of Additional Warranty Packages

Patients may purchase additional warranty packages for different types of services:

• Warranty & Dental Care Packages: Must be purchased before starting treatment or immediately after completing treatment.
• Warranty Package (Crown & Implant fixture): Valid for 1 year (can be used twice). Includes: Scaling, X-ray imaging, treatment warranty.

Package Options:

• 5-Year Package: Pay 20% of the total implant treatment fee (including fixture, abutment, crown, temporary tooth, bone graft, membrane graft, etc. – based on the total fee before discounts).
• 10-Year Package: Pay 60% of the total treatment fee.
• Lifetime Package: Pay 80% of the total treatment fee.
• Care Package: 1,400,000 VND – For patients: 600,000 VND, Includes: Scaling, X-ray imaging.

The warranty applies when:

• Patients undergo regular scaling and dental cleaning every 4–6 months.
• Patients attend follow-up appointments as scheduled by the treating doctor.
• The patient maintains oral care according to the dentist’s instructions.
• Patients use a night guard if bruxism (teeth grinding) is present.
• Patients promptly inform their dentist if experiencing pain, swelling, or bleeding.
• For international patients: the same warranty conditions apply, but if they cannot return to Vietnam, they may attend check-ups in their home country, send X-ray films via email or Viber, and receive oral care guidance remotely.

Attending scheduled follow-ups with the dentist is a mandatory condition for maintaining implant warranty.

The warranty will not apply if:

• The patient does not return to Nhan Tam Dental Clinic.
• The patient misses follow-ups or fails to follow the dentist’s instructions. Each missed annual follow-up will reduce the warranty period by 1 year.
• The patient eats excessively hard foods or exerts excessive biting force.
• The patient has poor oral hygiene before or after treatment.
• The patient smokes heavily.
• The patient develops new periodontal (gum) disease.
• The patient has systemic health conditions that negatively affect oral health (e.g., endocrine disorders, cardiovascular disease, diabetes, etc.).
• The patient has undetected allergies or underlying health issues before dental procedures.
• Damage is caused by severe stress (e.g., bruxism, clenching).
• The patient does not use a protective night guard when prescribed for bruxism.
• Damage results from accidents or trauma requiring restoration.
• The patient does not promptly report dental problems or seeks repair elsewhere. The clinic will not cover costs or responsibility in such cases.
• For crowns: warranty does not cover color or shape changes once the permanent restoration has been cemented and approved by the patient.
• Implants placed at another clinic: warranty applies only to the prosthetic crown at Nhan Tam Dental Clinic, not to the implant fixture itself.
• Patients should avoid chewing excessively hard foods after implant surgery.

Special Cases – Reduced Warranty Coverage

• Patients with poor oral hygiene (periodontitis, heavy smoking): Warranty for surgery reduced to 50%; crown warranty remains the same, as the success rate is lower. Patients must purchase an additional package to extend coverage.
• Patients with underlying systemic diseases affecting treatment results (diabetes, osteoporosis, autoimmune diseases such as lupus erythematosus, organ transplant patients using immunosuppressive drugs, etc.): Warranty for surgery reduced to 50%; crown warranty remains the same. Patients must purchase an additional package to extend coverage.

III. Dental Crown Warranty Policy & Conditions

Dental Crown Warranty Policy

Warranty is valid when:

• The patient attends Nhan Tam Dental Clinic in person.
• The dental crown is cracked, broken, deformed, or has fallen off.
• The patient presents a valid warranty card within the warranty period.

Warranty is not valid when:

• The patient does not follow the dentist’s instructions.
• Poor oral hygiene or frequent smoking.
• Failure to attend periodic check-ups every 6 months.
• Teeth grinding habit without using a prescribed night guard.
• Trauma caused by accidents or sports activities.
• Chipping due to chewing excessively hard foods such as bones, ice, or bottle caps.
• Natural wear and tear of enamel or teeth.
• Root canal treatment required but not included in the original treatment plan.
• Request to change shape or color after the crown has been permanently fixed with prior agreement.
• Development of new systemic diseases negatively affecting oral health after treatment, such as diabetes, osteoporosis, cancer radiotherapy, epilepsy, etc.
• Intervention by another dentist/clinic before returning to Nhan Tam Dental Clinic.
• Crowns made at the patient’s request (despite the dentist’s advice that they may be less durable, prone to loosening, or breaking).

IV. Orthodontic Service Warranty Policy & Conditions

Warranty Period: 2 years from the date of bracket removal.

Warranty is valid when:

• The patient attends Nhan Tam Dental Clinic in person.
• Relapse occurs after treatment.
• The patient presents a valid warranty card within the warranty period.

Warranty is not valid when:

• The patient does not wear the retainer regularly as instructed by the dentist.
• The patient fails to attend periodic check-ups every 6 months.

Notes: If relapse occurs during the warranty period, retreatment will be provided completely free of charge. After the warranty period, a 10% discount on retreatment fees will be applied. If a new retainer is required, a 20% discount on the retainer fee will be applied.

V. Dental Filling Warranty Policy & Conditions

Warranty Period: 6 months from the date of service. If the filling falls out within the warranty period, it will be redone completely free of charge.

Warranty applies when:

• The patient attends Nhan Tam Dental Clinic in person.
• The filling has fallen out.
• The patient presents a valid warranty card within the warranty period.

Warranty does not apply when:

• The patient fails to maintain oral hygiene as instructed by the dentist.
• The patient bites/tears excessively hard food (e.g., bones, ice, bottle caps, etc.).
• The filling is located on the biting edge and becomes chipped.
• Filling is done upon patient’s request (despite the dentist’s warning of possible detachment).

VI. Removable Denture Warranty Policy & Conditions

Warranty Period: 1 year from the date of service.

Warranty applies when:

• The patient attends Nhan Tam Dental Clinic in person.
• The removable denture becomes loose, the base is cracked, or clasps are broken, etc.
• The patient presents a valid warranty card within the warranty period.

Warranty does not apply when:

• The patient fails to follow the dentist’s instructions.
• Damage occurs due to biting excessively hard food (e.g., bones, ice, bottle caps, etc.).

Notes: If the denture becomes loose, chipped, cracked, or has broken clasps during the warranty period, repairs will be carried out completely free of charge. After the warranty period, patients will receive a 10% discount on repair or treatment costs.
